Health history and consent, before the visit.

New clients get a secure link in their booking confirmation and complete the form on their phone, with no account and no password. You choose which sections they see: turn optional sections on or off in Settings, while the required identity and consent sections stay put. A submitted form is signed and locked; if something changes you add a dated addendum, and the original is never altered. You can also Send or Open a client's intake straight from the appointment, and the dashboard flags who still needs to fill one in.

A few honest limits for v1. The templates are fixed: you switch sections on or off, but you cannot add custom fields yet. Delivery is by email only. You remain the custodian of every response, and the template is a convenience, not legal, clinical, or regulatory advice, so review it against your own obligations. Nothing goes out to a client until you have opened the Intake tab in Settings at least once, so a form is never sent before you have seen it.

The client sees a short, mobile-first form: their name and contact details already filled in from the booking, one section at a time with a progress bar, and questions that expand only when they apply. They sign by drawing or typing, get a plain thank-you when they submit, and can reopen the link to see it is already done. A link that has expired says so.
